On June 21, a Taiwan Railways express train derailed after colliding with a debris flow in Hualien. Following an investigation report, Taiwan Railways announced disciplinary actions with Chairman Du Wei taking greater responsibility by voluntarily offering to step down to the Minister of Transportation. In addition, the General Manager and 7 others received warnings or reprimands, while 4 staff members were commended. The incident prompted discussions on the importance of safety reforms in Taiwan Railways, including risk management and execution methods, with further reviews and proposals to be presented to the board soon.
